Abstract Minimally invasive approaches are increasingly being used for the correction of abdominal wall hernias. Although laparoscopy presents as a greater technical challenge compared to classical techniques, once learning curve is overcome, advantages multiple: less postoperative pain, shorter hospital stay, faster return daily/professional activities and lower rate surgical site infection. The authors present case 55yo women with history type 2 diabetes mellitus peripheral venous insufficiency. No previous surgeries. patient was referred an appointment in Ambulatory Surgery Centre evaluation primary midline hernia. Symptoms were worsening last 6 months. episodes complications. Preoperative included abdominopelvic computed tomography, which revealed hernia sac containing only fatty tissue (no intestinal loops present) 19 mm hernial neck supra-umbilical median location. other defects evident. proposed laparoscopic approach - extended totally extraperitoneal (eTEP) ambulatory setting. surgery post-operative period (9 months) date went without intercurrences. video aims demonstrate main steps technique applied tertiary centre years experience surgery. With this paper pretend that may become gold-standard soon know-how respective aquired. Until then, these cases should continue be specialized centres.
